Forms

• Format request for change 
  May be submitted electronically, except in the case of a change in the bank account number. An original signature is then required. >> Budget shifts: a request for change only needs to be filled out for shifts between budget items greater than 20%.
• Format for narrative progress report
  Reporting moments: period up till 31 December 2009: deadline 1 March 2010. Subsequently, always on 1 September for the first semester of the year and on 1 March for the second semester of the previous year. Please send the reports electronically and via postal mail to the addresses below. 
• Format for financial progress report
  Same reporting moments 
• Advance request form 
  Please note, always submit this form in Excel format and send the signed original via postal mail.
• Example wage rate calculation (only for first advance request)
• Example auditor's certificate for wage rate calculation (only for first advance request) 
  Please note: the wage rates in the advance request and the auditor's certificate must exactly correspond to those mentioned in the original budget. 
• Audit protocol for activities grants 
  Only applicable after project termination >> Only to submit if the subsidy amount exceeds € 125,000 3.  

Calendar 

• 18 December 2009: last opportunity to submit an advance request for 2009 1 March 2010: deadline for first narrative and financial progress reports (for the period through 31 December 2009) 1 September 2010: deadline for second narrative and financial progress reports (for the period 1 January – 30 June 2010)
